Have insurance or Medicare details ready, if they apply to you.

Plan to finish in one sitting — answers aren’t saved until you submit.
How your answers are handled
When you press Submit, your completed forms are sent to the office,
and a copy is downloaded to your device — keep it for your records or
bring it to your appointment.
